Auger-Aliassime set up Sinner in his run to the US Open final, hitting hard early on to rush Sinner and occasionally match him shot for shot.
However, the injury while leading 6-5 30-0 had an immediate impact; Auger-Aliassime struggled to move and sent his shots long as he tried to get out of trouble.
Having drawn just once in 10 service games, Sinner did not give up and sealed victory with an ace before being serenaded by her own fans.
He and rival Alcaraz are vying for the year-end title in Turin.
The Sinner must defend their title to retain the top spot and hope Alcaraz loses their group match and fails to reach the final.
Alcaraz, who is in the opposite group of Sinner, started his campaign with a decisive victory against Alex de Minaur on Sunday.
