‘Systemic’ failures led to culture of abuse and neglect

More than 700 pages long, listing 106 recommendations, the report proposes a comprehensive program of reform in response to a deep catalog of failures, widespread abuses, systematic leadership failures and the mishandling of a review of critical CCTV evidence.
