Final report on how the 332-metre Maersk container ship crashed into the much-loved Perth sailing ship Leeuwin transferred.

An investigation announced today by the Australian Transport Safety Bureau found that communication between the helmsman and the bridge was poor at the time of the accident and a number of risk controls required by the Port of Fremantle were not properly implemented.
The report stated that the critical factor in the accident was that one of the ship’s pilots was on the phone at the time.
“Various maritime casualty investigations, both in Australia and overseas, have highlighted mobile phone use as possible contributing factors during critical stages of transit,” the report said.

The report allowed the Leeuwin Ocean Adventure Foundation to better understand the sequence of collision events that had a significant impact on STS Leeuwin II and the Foundation and disrupted the LOAF’s cruise schedule.
LOAF CEO Lawson Dixon said the company was grateful that the two workers injured in the incident were on the road to recovery.
“We are grateful to the ATSB for their extensive investigation, which has provided a clearer understanding of the events leading up to the accident,” he said.
“Equally, we are encouraged that the Ports of Fremantle acted quickly to put appropriate protocols in place to help prevent any recurrence.
“Leeuwin is currently in the final stages of repair and we look forward to serving the Western Australian community again in 2026.”
