Fair Isle is located in the middle of the Northern Islands of Orkney and Setland.
The existing nurse of the island is leaving later this month.
NHS Setland now advertises to take its place and describes it as a “unique opportunity to be an integral part of the island life”.
The fair, which hosts about 50 people, is famous for its Isle, knitwear, wild life and bird observatory.
It is only three miles long (4.8 km) and one and a half miles wide.
The successful candidate will be responsible for guest tourists as well as all the population.
Eileen Thomson, one of the current inhabitants of Adil Adil Isle, believes that there may be a great movement for one of them.
“I think it may be the most surprising opportunity for one of the right types that come to Fair Isle,” he said to the BBC Good morning Scotland Program.
He said that the outgoing nurse was leaving this week and would be a cover period.
“It is always a little concern not to have someone here because we’re the farthest settlement island in the country.”

Eileen Thomson, “You may be dealing with all kinds of illnesses.” He said. “There will be quieter times and there will be really busy times, he has someone who likes to show some diversity in his work.
He continued: “You are the only person on the island as a medical expert. We are supported by a doctor surgery in the Setland mainland.”
Sumburgh -based coast security rescue helicopter doubles as the ambulance of the island, but the nurse went for everything.
He continued: “We have been lucky that we have incredible nurses for the last few years, we always like to invite new people to the community.”
I’m looking for a challenge
“The most beautiful little island” describes Fair Isle, whom he says is one of the best places to identify wild life, known for nature.
“Of course, we are famous with our knitwear, so everyone who had such an interest would find an incredible house here.
“But what we like to be known is that we are a really powerful small community. We can only be 50 people, but we are an incredibly powerful, hardworking community. We are a very social group.”
He said that the island life did not fit everyone and had difficulties.
“We’re in the distant north, the weather is not so big in winter, transportation is not so easy and you must be a really confident clinician. You are the person who looks at everything, you are the man on the ground.
“For an experienced person, for someone looking for a challenge – but a great challenge – I think it can be a really perfect role.”
Applications will close at the end of this month.
