VAR errors increase – every mistake so far in 2025-26 Premier League

The KMI Panel meets every week to review every decision, from goals to penalties and red cards.

It has five members: three former players and coaches, plus one representative each from the Premier League and Professional Match Officials (PGMO).

The KMI Panel was created by Premier League clubs in 2022 with the aim of taking performance statistics out of the hands of the refereeing committee.

More importantly, it is not intended to look at every incident the way any fan or manager would. It takes into account the rules of the game and, more importantly, how the Premier League expects its games to be officiated.

Therefore, the list of errors will not be as extensive as fans expected.

The KMI Panel first looks at the decision of the on-field referee team and then the VAR.

Because VAR operates at a higher threshold, there are times when the KMI Panel can say – loud and clear – that the referee was at fault but the VAR was right not to intervene.

The KMI Panel also takes into account missed or misrepresented second yellow cards.
