In 2012, the scrapped sentences imprisoned thousands of people without the date of release, and some served 22 times longer than their original tariffs.
A prison observer said that the prison staff had missed the key warning signs before Mr. GADD on March 9, 2023, and should do more to recognize the risk of suicide and self -harm for IPP prisoners.
His death came a month after Dominic Raab then rejected the advice from the Justice Committee of Justice to annoy all remaining IPP prisoners.
Former Conservative Deputy Sir Bob Neill, the chairman of the Justice Committee, said that the government had missed the opportunity to correct a mistake that led to about 3,000 people behind ”.
A report of Adrian Usher’s prisons and probation ombudsan (PPO), published this week, found that Mr. Gadd, who was wounded to cause severe physical damage, was the story of previous suicide attempts and suffered from anxiety and depression.
A risk assessment in 2021 warned that he had suicidal thoughts when he felt that he would need to monitor continuously, and that he was usually withdrawn and socially avoided.
In the report, many of these signs were appeared in the days before his death: “He didn’t go to work, he spent more time in his cell, and his behavior to his friends was characterless.”
The three prisoners on the wing told the staff that they were worried about him and said that “more weight” should be given in the light of IPP status.
Mr. GADD also waited for three months to approve the transfer of the Prison to the prison after being recommended for the Conditional Evacuation Board of the Justice Secretary. Ombudsman said it would be a source of “anxiety and disappointment”.
Mr. GADD was found unresponsive in his cell at 8 am on March 9 in the morning and was pronounced deadly after a while.
Mr. Usher concluded that HMP Bristol, a category B acceptance prison, was a “inappropriate place for Mr. GADD, because he could not offer him the risk reduction job or mental health support.
HMP had previously spent a year in Leyhill open conditions, but returned to closed conditions based on the “ear fountain evidence of ear” of another prisoner who claimed that he had heard that he had threatened to stab one.
In the report, Mr. GADD felt that this was a malicious accusation of a prisoner who was bullying, but his statement was never investigated by the prison staff.
HMP had met only a key worker in seven months of his held in Bristol, and later in a report of that year, he was described by the HM Prisons Chief Inspector as one of the most safe prisons of the country ”.

In September of that year, the prisons Ombudsman issued a special newsletter after the “anxious increase önemli in the deaths of self -deaths among those who served IPP sentences.
At the IPP Committee, the founding partner, Shirley Debono, participated in a protest outside the Bristol prison after Mr. Gadd’s death.
When Mr. Raab rejected the anger, the Minister warned the ministers that the IPP prisoners’ hopes were “broken” despite a large report that calls the ministers to appoint a expert committee to look at the process.
“I think the damage said Dominic Raab would not annoy these prisoners,” he said. Independent.
The government is considering new suggestions to give a two -year release date of the next conditional release of all IPP prisoners of the former Lord Chief John Thomas and Howard Penal Reform League.
A prison service spokesman said: “Our thoughts remain with those who know Keith Gadd and received supported to better support the IPP prisoners in HMP Bristol.
“We continue to work with organizations and campaign groups to ensure that the abolition of these sentences is true and still to support those who serve their punishments.”
