NTSB releases preliminary report on UPS plane crash

A man seriously injured in the violent crash of a UPS cargo plane in Louisville, Kentucky, has died more than seven weeks later, bringing the total number of deaths in the crash to 15.
Louisville Mayor Craig Greenberg stated that the person who was seriously injured in the accident died on Thursday and identified the victim as Alain Rodriguez Colina.
“It is with great sadness that I learned of the passing of Alain Rodriguez Colina,” Greenberg said. wrote in an X post Thursday evening. “Alain is the 15th victim of the UPS Flight 2976 crash. He was severely injured during the crash and passed away early this Christmas Day. May Alain’s memory be a blessing.”

Kentucky Governor Andy Beshear reacted to X’s updated death toll, urging people to “pray for these families today and in the days, months and years to come” to know they are not alone and that they are loved.
Colina’s death marks the latest development in one of the deadliest aviation disasters in the United States this year.
On November 4, UPS Flight 2976 crashed minutes after leaving Louisville International Airport, crashing into a nearby industrial area and killing three crew members and 11 people on the ground.
In the preliminary report prepared by the National Transportation Safety Board (NTSB), it was stated that the left engine of the cargo plane going to Honolulu was separated from the plane. It burst into flames shortly after takeoff.

Black box data shows the plane reached only 30 feet above the ground before impact.
According to the NTSB report, airport surveillance footage shows that shortly after the plane turned, the left engine and pylon separated from the wing and a fire broke out in the left engine.
The left engine then hit the ground and a fire broke out near the left pylon mount on the wing, continuing until the plane crashed into a nearby warehouse and two buildings, the report said.

Investigators also added that the rear portion of the left pylon broke, causing the engine to separate, and that they found small cracks that grew over time around the bolt holes in the mount until they failed under normal stress.
